The sleep problems can have different causes such as stress, shift work, various illnesses, etc. In the long run, sleep problems or insomnia can lead to poor physical and mental performance, irritability, difficulty concentrating, headaches and poor general mood.

VIVINOX Sleep sleeping tablets strong contain diphenhydramine, an H1 antihistamine, which causes drowsiness. The tablets can be taken by adults for a short time about 15 to 30 minutes before going to bed.

How to use?
Take the medicine with liquid (e.g. 1 glass of water).

Duration of use?
The medicine should not be used for longer than 2 weeks. As a rule, however, the duration of treatment should be as short as possible and only last a few days. If symptoms persist for longer, you should consult your doctor.

Overdose?
In the event of an overdose, drowsiness, impaired consciousness, hallucinations and cardiovascular disorders may occur. If you suspect an overdose, contact a doctor immediately.

How does the active ingredient of the medicine work?

The active ingredient blocks receptors for a specific messenger substance called histamine. This blockade suppresses histamine-induced vasodilation and increased blood flow in the smallest hair vessels (capillaries). It also combats nausea via the same mechanism and has a calming and sleep-promoting effect.

What should you bear in mind?
— The ability to react may be impaired even when used as intended, especially in higher doses or in combination with alcohol. Pay particular attention when driving or operating machinery (including household machinery) that could cause injury.
— Avoid excessive UV radiation, e.g. in solariums or during extended sunbathing, because the skin reacts more sensitively during the use of the medicine.
— Sudden discontinuation may cause problems or discomfort. Treatment should therefore be stopped slowly, i.e. by gradually reducing the dose. It is best to seek advice from your doctor or pharmacist.
— Caution: If you use the medicine for a long time, the body may become accustomed to it and you may need an increasingly higher dosage in order to feel an effect.
— Take care if you are intolerant to lactose. If you have to follow a diabetes diet, you should take the sugar content into account.

Vivinox is an antihistamine with anticholinergic and sedative effects. It competes with histamine for H1-receptor sites on effector cells in the GI tract, blood vessels and respiratory tract.

Vivinox has anti-histaminic (H1-receptor), anti-emetic, anti-vertigo and sedative and hypnotic properties . The anti-histamine action occurs by blocking the spasmogenic and congestive effects of histamine by competing with histamine for H1 receptor sites on effector cells, preventing but not reversing responses mediated by histamine alone . Such receptor sites may be found in the gut, uterus, large blood vessels, bronchial muscles, and elsewhere . Anti-emetic action is by inhibition at the medullary chemoreceptor trigger zone . Anti-vertigo action is by a central antimuscarinic effect on the vestibular apparatus and the integrative vomiting center and medullary chemoreceptor trigger zone of the midbrain .

How Vivinox works

Vivinox predominantly works via the antagonism of H1 (Histamine 1) receptors . Such H1 receptors are located on respiratory smooth muscles, vascular endothelial cells, the gastrointestinal tract (GIT), cardiac tissue, immune cells, the uterus, and the central nervous system (CNS) neurons . When the H1 receptor is stimulated in these tissues it produces a variety of actions including increased vascular permeability, promotion of vasodilation causing flushing, decreased atrioventricular (AV) node conduction time, stimulation of sensory nerves of airways producing coughing, smooth muscle contraction of bronchi and the GIT, and eosinophilic chemotaxis that promotes the allergic immune response .

Ultimately, diphenhydramine functions as an inverse agonist at H1 receptors, and subsequently reverses effects of histamine on capillaries, reducing allergic reaction symptoms . Moreover, since diphenhydramine is a first-generation antihistamine, it readily crosses the blood-brain barrier and inversely agonizes the H1 CNS receptors, resulting in drowsiness, and suppressing the medullary cough center .

Furthermore, H1 receptors are similar to muscarinic receptors . Consequently, diphenhydramine also acts as an antimuscarinic . It does so by behaving as a competitive antagonist of muscarinic acetylcholine receptors, resulting in its use as an antiparkinson medication .

Lastly, diphenhydramine has also demonstrated activity as an intracellular sodium channel blocker, resulting in possible local anesthetic properties .

Dosage

Adult-

  • Most allergic conditions: 25-50 mg three times a day with a further 50 mg at night.

Children-

  • 1 to 5 years of age: 5 mg i.e., 2.5 ml of elixir 4 times a day
  • More than 6 years of age: 10 mg i.e. 5 ml of elixir 4 times a day

Side Effects

Side effect includes sedation, dizziness, tinnitus, fatigue, ataxia, blurred vision, diplopia, euphoria, and epigastric discomfort.

Toxicity

Overdose is expected to result in effects similar to the adverse effects that are ordinarily associated with the use of diphenhydramine, including drowsiness, hyperpyrexia, and anticholinergic effects, among others . Additional symptoms during overdose may include mydriasis, fever, flushing, agitation, tremor, dystonic reactions, hallucinations and ECG changes . Large overdose may cause rhabdomyolysis, convulsions, delirium, toxic psychosis, arrhythmias, coma and cardiovascular collapse . Moreover, with higher doses, and particularly in children, symptoms of CNS excitation including hallucinations and convulsions may appear; with massive doses, coma or cardiovascular collapse may follow .

Although diphenhydramine has been in widespread use for many years without ill consequence, it is known to cross the placenta and has been detected in breast milk . This medication should therefore only be used when the potential benefit of treatment to the mother exceeds any possible hazards to the developing fetus or suckling infant .

Pharmacokinetic studies indicate no major differences in the distribution or elimination of diphenhydramine compared to younger adults . Nevertheless, diphenhydramine should be used with caution in the elderly, who are more likely to experience adverse effects . Avoid use in elderly patients with confusion .

The results of a review on the use of diphenhydramine in renal failure suggest that in moderate to severe renal failure, the dose interval should be extended by a period dependent on Glomerular filtration rate (GFR) .

After intravenous administration of 0.8 mg/kg diphenhydramine, a prolonged half-life was noted in patients with chronic liver disease which correlated with the severity of the disease . However, the mean plasma clearance and apparent volume of distribution were not significantly affected .

LD50=500 mg/kg (orally in rats). Considerable overdosage can lead to myocardial infarction (heart attack), serious ventricular dysrhythmias, coma and death.

Precaution

Caution should be exercised with patients in whom drowsiness is undesirable e.g., drivers, machine operators. Concomitant consumption of alcohol or central nervous system (CNS) depressants will potentiate drowsiness.

Interaction

Vivinox administration significantly reduces the absorption of the antituberculous agent para-aminosalicyclic acid (PAS) from the gastrointestinal tract. CNS depressants may potentiate the sedative action of Vivinox. Anticholinergic drugs may potentiate Vivinox’s anticholinergic side effects.

Food Interaction

  • Avoid alcohol.
  • Take before a meal. Take 30 minutes before a meal or 30 minutes before departure for motion sickness.

[Moderate] GENERALLY AVOID:

Use of anticholinergic agents with alcohol may result in sufficient impairment of attention so as to render driving and operating machinery more hazardous.

In addition, the potential for abuse may be increased with the combination.

The mechanism of interaction is not established but may involve additive depressant effects on the central nervous system.

No effect of oral propantheline or atropine on blood alcohol levels was observed in healthy volunteers when administered before ingestion of a standard ethanol load.

However, one study found impairment of attention in subjects given atropine 0.5 mg or glycopyrrolate 1 mg in combination with alcohol.

Alcohol should generally be avoided during therapy with anticholinergic agents.

Patients should be counseled to avoid activities requiring mental alertness until they know how these agents affect them.

Vivinox is widely distributed throughout the body, including the CNS . Following a 50 mg oral dose of diphenhydramine, the volume of distribution is in the range of 3.3 — 6.8 l/kg .

Elimination Route

Vivinox is quickly absorbed after oral administration with maximum activity occurring in approximately one hour . The oral bioavailability of diphenhydramine has been documented in the range of 40% to 60%, and peak plasma concentration occurs about 2 to 3 hours after administration .

Half Life

The elimination half-life ranges from 2.4-9.3 hours in healthy adults . The terminal elimination half-life is prolonged in liver cirrhosis .

Clearance

Values for plasma clearance of a 50 mg oral dose of diphenhydramine has been documented as lying in the range of 600-1300 ml/min .

Elimination Route

The metabolites of diphenhydramine are conjugated with glycine and glutamine and excreted in urine . Only about 1% of a single dose is excreted unchanged in urine . The medication is ultimately eliminated by the kidneys slowly, mainly as inactive metabolites .

Pregnancy & Breastfeeding use

Category B: There are no adequate and well controlled studies in pregnant women using diphenhydramine hydrochloride. Therefore, diphenhydramine hydrochloride should be used in pregnancy only if clearly needed. Vivinox hydrochloride has been reported to be excreted in breast milk and thus, use of diphenhydramine hydrochloride in lactating mother is not recommended.

Contraindication

Known hypersensitivity to Vivinox Hydrochloride, Ammonium chloride is contra-indicated in presence of impaired hepatic or renal function.

Acute Overdose

Symptoms: Impaired consciousness; psychosis, seizures, antimuscarinic symptoms (e.g. mydriasis, tachycardia, tachyarrhythmias), resp failure, rhabdomyolysis; acute delirium with visual and auditory hallucination (topical).

Management: Supportive and symptomatic treatment. Convulsions and marked CNS stimulation may be treated with IV diazepam.

Дозировка vivinox® ночного сна драже
  • Принимайте препарат всегда точно в соответствии с инструкцией в данном листке-вкладыше. Пожалуйста, не спросите у своего врача или фармацевта, если вы точно знаете.
  • Если врач не назначил другое, обычная доза составляет:
    • Для взрослых и детей старше 12 лет для лечения Беспокойства 1 раз в день 1 покрынные таблетки (драже), для лечения бессонницы нервных условного покрынные 1 таблетке (драже) 1/2 до 1 часа перед сном.
  • Пожалуйста, поговорите со своим врачом или фармацевтом, если у вас сложилось впечатление, что действие этого лекарственного средства слишком сильным или слишком слабым.
  • Длительность применения
    • Длительность применения препарата не является в принципе ограниченным. При Остановке жалоб через 2 недели или при обострении нужно принимать посоветовавшись с врачом.

Diphenhydramine (brand: Vivinox) is an antihistamine that reduces the effects of natural chemical histamine in the body. Histamine can cause symptoms such as sneezing, itching, watery eyes and runny nose.

Diphenhydramine (Vivinox) is used to treat

  • Sneezing,
  • Watery eyes,
  • Runny nose,
  • Skin rash,
  • Hives,
  • Itching, and
  • other cold or allergy symptoms.

Diphenhydramine is also used to treat

  • Motion sickness,
  • To induce sleep, and
  • To treat certain symptoms of Parkinson’s disease.

Maximal effect is typically around two hours after a dose, and effects can last for up to seven hours.

Vivinox Mechanism

Diphenhydramine (Vivinox) competes with free histamine for binding to HA receptor sites. This antagonizes the effects of histamine on HA receptors, which leads to a reduction in negative symptoms caused by the binding of the histamine HA receptor.

This drug blocks some natural substance (histamine) that your body forms during an allergic reaction. Its dehydrating effect on symptoms such as watery eyes and nasal discharge is caused by the blockage of another natural substance (acetylcholine).

Following are some of the side effects associated with the use of vivinox

  • Constipation,

Dosage and administration

  • Available in Oral, IV, IM, rectal, topical, intranasal, in the conjunctival sac.
  • Usual Oral dose for adults is 30-50 mg 1-3 times / day. The treatment course is 10-15 days.
  • IM in doses of 50-250 mg;
  • IV in drip – 20-50 mg.
  • When oral administered single dose for children under 1 year – 2-5 mg;
  • Usual dose for 2 to 5 years of age- 5-15 mg; of 6 to 12 years – 15-30 mg.
  • Externally applied 1-2 times / day.

Vivinox interactions with Other Drugs

Certain Drugs Increase or decrease the effects of other drugs when use concomitantly.

Diphenhydramine (Vivinox) simultaneous use with Mono Amine Oxidase inhibitors increase the anticholinergic activity of diphenhydramine.

When applied simultaneously increases the effects of ethanol and drugs that depress the central nervous system.

The antagonistic interaction observed with a joint appointment with psychostimulants.

Intensifies anticholinergic effects of drugs with anticholinergic activity.

Reduces the effectiveness of apomorphine as an emetic in the treatment of poisoning.
